LMH Health strives to provide the highest quality in our patient care. In an initiative to provide patient first care, we ask our patients to complete a survey about their experiences. Your ratings and reviews provide us with feedback to assess our quality of care and address areas that need improvement.

You have recently visited one of our LMH Health providers. After your appointment, you will receive an invitation to complete our survey via text and email. The survey asks you to rate and review your visit. You will receive one survey in a 90-day period. We send these surveys to receive your feedback, which allows us to improve our patient experience.

Only LMH Health patients who received our services. The survey will be sent to your email and in a text message. Don’t worry, it is not spam and you will not be charged for visiting the site.

Whether the review is positive or negative. All reviews are screened so no patient health information is publicly displayed and no reviews with potentially libelous or profane words are shown. Ratings and reviews that contain this information will not be posted. For your safety and protection, all of your personal information has been removed from the survey. This includes your name and any other potential identifiers.
Not all providers have ratings because…
Ratings and reviews are only available for providers who are employed by LMH Health. Additionally, our providers must have a minimum of 30 patient reviews for their scores to be posted. We have set this minimum for our patients to have the most accurate and current reviews. Reviews shown are from the previous 12-month period.

Each and every review is from an LMH Health patient. This differs from other online reviews because the respondents are verified.
