- View your lab results and imaging results.
- View your visit notes, summaries and vaccination records.
- Request and manage your appointments.
- Request medication refills.
- Securely message your doctor 24/7.
If you are being tested at LMH Health for COVID-19:
Receive immediate notification of your results through My Patient Portal as soon as your test is processed. Ask a member of our care team to register you up for My Patient Portal when you schedule testing, surgery or any other procedure at our hospital or doctor's offices.

Who can register?
If you’re a patient at LMH Health or one of our affiliated clinics and are 18 or older, you can register online for My Patient Portal. Parents also can also register their children who are younger than 13. Learn more about how to register.
Connect your health management apps
Apple Health connects with My Patient Portal so you can easily access your electronic medical record (EMR). My Patient Portal may be available to connect to other apps.
