Patient and Family Advisory Council (PFAC)
At LMH Health, the voice of the patient matters.
If you've been a patient at LMH Health, or a family member or caregiver of a patient, you may have the opportunity to help us improve our services and make a difference in the lives of other patients and families. The Patient and Family Advisory Council (PFAC) is a diverse group of volunteers who meet regularly to recommend ways to enhance the patient experience. As your partner for lifelong health, your feedback is important to us and helps us continue to improve our patient-centered care.
To become a member of the PFAC, you must:
- Be a former patient, or a family member or caregiver of a former patient
- Share LMH Health’s commitment to excellence in patient-centered care
- Have a positive approach and the ability to share and see different points of view
- Be able to serve two hours a month for a maximum of two years
- Attend a Patient and Family Advisor Orientation
