Appointment Hours

We offer extended hours from 7:30 a.m. to 5 p.m., Monday-Friday, excluding holidays.

Our primary care providers offer same-day appointments. Appointment times may vary according to your physician’s schedule. We are conveniently located at Sixth Street and Folks Road in the Old Oak Medical Center, with easy access to I-70.

My Patient Portal

My Patient Portal is an innovative system that enables our patients to send us messages and request basic lab test results, health summaries, electronic health records and appointments – all with secure access wherever Internet service is available.

To get started, complete this Patient Portal Agreement and bring it in at your convenience or during your next appointment. Our friendly staff will provide your login and other helpful information.
