What Does the Palliative Support Services Consulting Team Do?

Physician/Nurse Practitioner

  • Your attending doctor is involved with your care in most cases
  • Make recommendations for management of complex symptoms
  • Discuss your medical care with other physicians involved, including your primary care provider
  • Ensure you and your family members understand your diagnosis and prognosis
  • Educate medical staff and other staff members about palliative medicine

Nurses

  • After talking with you and your family, develop an initial palliative care plan
  • Assist with managing difficult symptoms
  • Advocate for you, your family members and caregivers on goals of care
  • Educate staff members about palliative care
  • Assure that all necessary disciplines are on the team

Social Workers & Spiritual Care

  • Help you and your family members cope with many issues
  • Assist with finding resources and discharge planning
  • Support your advance care planning
  • Facilitate meetings you and your family members have with members of the palliative care team
