Make a gift that will support LMH Health in the long-term. Gifts to the LMH Health Foundation general endowment are invested in perpetuity, and only the annual investment earnings are used in support of LMH Health.

Falling Forward Foundation at LMH Health

Many people do not receive adequate rehabilitation to fully recover from catastrophic medical events. LMH Health is a proud partner of the Falling Forward Foundation, which makes grants to rehab centers in order to allow patients to continue their therapy, recover to their full potential and move forward in their lives.

Most medical insurance plans cover only 20 – 30 visits to a rehabilitation center. That’s not adequate for people who are recovering from catastrophic medical issues such as:

  • Strokes
  • Traumatic brain injuries
  • Spinal cord injuries

As a result, those patients generally stop recovering once their insurance runs out. Through a partnership with Falling Forward Foundation, patients have an opportunity to continue therapy and recover to their full potential.

How To Apply for Help

If you are currently in rehab and about to reach your therapy cap, we may be able to help. All decisions on how to allocate Falling Forward® funds are made by rehab professionals at LMH Health. To initiate the process for consideration:

  • Your doctor or current therapist should contact LMH Health Therapy Services to refer you for consideration.
  • You will be evaluated by a therapist who will assess your potential for continued improvement.
  • You will be asked to complete an application, including disclosure of household income.

Catch a Break logo

The Catch a Break Fund

The Catch a Break Fund at LMH Health Foundation helps cancer patients and survivors in our community pay for day-to-day needs like medications, car repairs, utilities and food. Your financial gift will allow the recipient to “catch a break” during one of the most difficult times in their life.

In 2007, Catch a Break started as a small grassroots organization to help a few women who were battling breast cancer. Cindy Sargent, a survivor herself, experienced the overwhelming burdens and wanted to help ease the path for others. With the help of friends and volunteers, Cindy aimed to assist fellow cancer survivors with the “cost of survivorship” during the holidays. She launched this program by adopting six families, providing gifts and cash assistance.

In 2010, Catch a Break joined the LMH Health Foundation family of charitable opportunities. With your support, this program continues to help survivors and their families each year.

How the program works

For Survivors

Support is given for day-to-day living expenses for food, utilities and rent. It is intended for survivors currently in or within one year of completing treatment with a financial need not to exceed $300. For more information and to apply, please contact the social workers at the LMH Health Cancer Center. 

For Donors

Gifts of any amount are welcome and will make a difference in the lives of local cancer patients and survivors. Please make a gift by visiting LMH Health Foundation’s donation page and selecting “Catch a Break” under Designation. This program could not exist without you.
